I understand this authorization is made in accordance with the Federal Communications Commission's rules regarding Local Number Portability under 47 U.S.C. § 251(b)(2) and related regulations. I certify that I am authorized to request number portability for the telephone number(s) listed and that all information provided is true and correct. I acknowledge that my current provider may charge early termination fees and/or outstanding balances for which I will remain responsible. I understand that service interruption may occur during the porting process. I acknowledge that this request will not change any existing contractual obligations I have with my current provider, and it is my responsibility to ensure those obligations are fulfilled separately.
